Find your prescription drug(s) in our Prices section, select the correct dosage and quantity, and add them to your 'Shopping Cart'.
Step 2:
When you have found all the products you require, click 'Checkout' to complete your order. Fill in the required Customer Agreement and Patient Profile completely and accurately, and submit your order to EarthMeds' mail order pharmacy network.
Step 3:
Fax a copy of your valid prescription(s) to EarthMeds along with our printable 'Prescription Fax Cover Sheet', found on our Faxable Forms page.

Go to EarthMeds' Faxable Forms page and print a 'New Patient Order Form'.
Step 2:
Complete the form accurately and as neatly as possible.
Step 3:
Fax a copy of the form along with your valid prescription to EarthMeds mail order pharmacy network. Include our 'Prescription Fax Cover Sheet' which can be found on our Faxable Forms page.
